Consent to Treatment

BioWave Regeneration does not ask you to sign a treatment consent on this website. Consent is reviewed with you, in person, at your first visit, after your $49 exam and before any treatment starts. This page explains what that means so there are no surprises when you arrive.

What you will be asked to read and sign in the office

• A treatment consent. It names the therapies we offer (focused shockwave, red laser therapy and transcutaneous pulsed radiofrequency nerve therapy), the area you are being treated for and the complaint you came in with, the most common side effect (mild, temporary soreness), the fact that results vary and no outcome is guaranteed, and your right to stop treatment at any time without affecting your right to future care.
• A written financial agreement, only if you and your provider agree on a treatment plan. It states what is included, the total price, the payment schedule, and the refund and cancellation terms for that plan. Nothing is charged for treatment until you have read and signed it.
• Our Notice of Privacy Practices, which explains how your health information is used and protected and how to ask for a copy of your records.

What you should tell your provider

Your full health history, including medications you take, any implants, recent surgery, and anything a doctor has told you about your condition. Bring a list if it helps. Your provider uses this to decide whether a therapy is appropriate for you; some conditions mean we will not treat.

Your rights

• You may ask questions at any point and take the forms home to read before signing.
• You may withdraw consent and stop treatment at any time.
• You may request copies of your records and of anything you signed.
• Nothing about your care is shared with anyone outside your treatment except as the law allows and as the Notice of Privacy Practices describes.
